Non-Tideway Fatality: Feb-2017 - Two fitters and an operator were using a scissor lift for access when a weld failed in one of the main supports, causing the scissor lift to collapse from a height of approx. 20 metres and resulting in the death of one fitter. Subsequent investigation revealed pre-existing cracks in the welding on a structural member and resulted in a temporary ban on the use of this equipment.

On 5th May 2016 MTS Cleansing Services Limited (MTS) were lifting a manhole using a hydraulic manhole lifter hired from Speedy Hire. During this operation one of the jacks became harder to operate. On the next operation of that jack, one of the four lifting keys attaching the lifter to the manhole failed. The investigation confirmed that the operatives were trained and had set up the lifter correctly. The equipment had all relevant LOLER and PUWER certification and had been given a pre-use check by the hire company in line with the manufacturer instruction supplied.

UPS unit explosion

A Production Technician was injured whilst installing a UPS at one of our sites. These injuries were so significant that the technician required hospital treatment.

The UPS being installed was a RIELLO Sentinel Pro UPS and battery box.

Whilst connecting the UPS to the battery box the production technician noticed a fault code (F60) appearing on the UPS. He then powered down the UPS & continued to disconnect the UPS from the battery box. Whilst carrying out this the UPS exploded.

A technician was called to attend a gas leak at a treatment works. When he and the second man donned their BA and he entered the room it was apparent that there was a small leak from the connection between the cylinder and the pig tail lead.



The pigtail and the valve connection had the outward appearance of being nearly new therefore, seeing no visible evidence of corrosion, he attempted to tighten the connection while the line remained live. Download

Recently on a Thames Water site there was a catastrophic failure of an Acetylene bottle resulting in the cylinder catching fire. The site was immediately attended by the Fire Services who set up a 200 metre exclusion zone for vehicles and pedestrians which impacted local businesses.

Safer lorry scheme

Transport for London’s ‘Safer Lorry Scheme’, came into force on 1 September 2015 in London.

Over recent years there has been a growing movement towards enhancing the safety controls on commercial vehicles within the capital. The Safer Lorries Scheme ensures that only lorries with basic safety equipment fitted will be allowed on London's roads. Any vehicle currently in service not having the required safety equipment will have to be retrofitted.

This includes construction vehicles, which are involved in a disproportionate number of fatal collisions involving cyclists and pedestrians.
